PART 6 – Annexes A–P Toolkit: The Complete Forms and Validation System of RSA

This is the most important part of the RSA series. While Enclosures 2–5 tell you how points are computed, the Annexes A–P tell you how HR and HRMPSB actually validate and justify every score and decision.

If it is not documented in an Annex, it does not legally exist in the RSA process.

This guide explains each Annex in plain language, tells you who fills it out, when it is used, and shows how HRMPSB uses it during deliberation.

Annex-by-Annex Explanation (Plain Language + Practical Tips)

Annex A – Posting of Vacancies

Used by: HRMO

Purpose: Legal transparency requirement.

What you must know:

  • All vacancies must be publicly posted.
  • No “secret hiring” is allowed.
  • Posting must include QS and required documents.

Annex B – Document Screening Checklist

Used by: HRMO

This Annex determines whether your folder is:

  • ACCEPTED
  • ACCEPTED WITH CONDITIONS
  • NOT ACCEPTED

Common reasons for NOT ACCEPTED:

  • missing license or eligibility
  • TOR/COE lacks signatures
  • L&D certificates with no hours
  • unverified experience

Annex C – Initial Evaluation Rating Sheet

Used by: HR evaluators or panels

This Annex captures your raw scores based on the appropriate Enclosure (2–5).

Important note:
Scores here are not final.
They must pass HRMPSB validation.


Annex D – BEI (Behavioral Events Interview) Rating Sheet

Used by: HRMPSB interview panel

Factors evaluated:

  • professional behavior
  • decision-making
  • problem-solving
  • ethics
  • communication
  • teamwork

Tip: Use STAR format to maximize clarity.


Annex E – Demo Teaching Rating Sheet (Teacher I only)

Evaluates:

  • content mastery
  • strategies & pedagogy
  • classroom management
  • learner engagement

Demo scores must match the official rubric. Inconsistencies require justification in Annex H.


Annex F – Document Verification Tool

This is the most feared annex—used to downgrade or zero out points if evidence is:

  • unclear
  • inconsistent
  • unverified
  • incomplete

Examples of downgrades:

  • L&D without hours → minimal points
  • Experience with no COE → not counted
  • Achievements with no proof → disallowed

Tip for applicants:
Assume HR will check every detail. Prepare a clean, well-organized folder.


Annex G – Final Score Consolidation Sheet

Used by: HRMO before HRMPSB deliberation.

This is the summary sheet showing:

  • demo scores
  • BEI scores
  • experience
  • L&D
  • education
  • performance

A single mistake here affects your ranking.


Annex H – HRMPSB Deliberation Report

The legal heart of the RSA process.

The HRMPSB must record:

  • final validated scores
  • justifications for score adjustments
  • unusual cases
  • decisions on ties
  • issues or conflicts
  • any applicant who withdrew or was disqualified

This protects the integrity of the ranking list.


Annex I – Final Ranking List

Signed by HRMPSB members. Contains:

  • names
  • final scores
  • rank numbers

This is the list the SDS uses.


Annex J – Recommendation to Appointing Authority

The HRMPSB formally recommends the top-ranked eligible applicants.


Annex K – Appointment Transmittal

Documents the appointment made by the SDS or Regional Director.


Annex L – Applicant Notification Form

This is how applicants are formally informed of their results.

Note: Applicants may request copies of their score sheets through proper channels.


Annex M – Protest / Appeal Template

Applicants use this if they believe:

  • scoring was incorrect
  • documents were not validated properly
  • procedures were not followed

Tip: The strongest appeals are evidence-based, not emotional.


Annex N – Protest Resolution Form

Used by the SDS or designated authority to formally resolve appeals.

All decisions must reference Annex H and validated evidence.


Annex O & Annex P – Monitoring & Compliance Forms

Used by the division/regional office to ensure the RSA process was followed.

These forms track:

  • timeliness
  • compliance with posting
  • proper use of forms
  • number of applicants
  • complaints filed
